Help for Galaxy S/Z15 please

I have a new Galaxy S (first time with this type of phone) and a new Exetel mobile plan (Z15).

1) If I call my Exetel DID does this call come under the Exetel to Exetel included calls? I thought it would have but on the billing no Exetel to Exetel calls are showing up, so does Exetel to Exetel mean only mobile to mobile?
2) From the Galaxy, normal phone calls, SMS and access to internet (via both wi-fi and APN) appear to be working OK. Using a Telstra phone I sent an MMS to the Galaxy. The message left the Telstra phone OK. On the Galaxy the phone the MMS appeared to arrive but it's status was always "downloading" and it never completed downloading. Also tried sending an MMS to the Telstra phone, the MMS status was shown as "uploading" and again it never completed, nothing arrived at the Telstra phone. Are there some settings required for MMS to work with the Galaxy/Z15 (can't believe that Telstra seems to "just work" and Exetel does not)?
3) Also tried to make a video call from the Galaxy phone to Telstra phone, and vice versa. In both cases just get a message saying call cannot complete. Are there any settings required to make this work?

No, if you read the exclusion conditions on the website, you will note this:

Calls from an Exetel mobile service to an Exetel landline or VoIP service are not classified as on-net ‘Exetel to Exetel’ calls and therefore excluded from that category of calls.

Yes - only Exetel mobile to Exetel mobile

We might have to check the sub-plan code provisioning. Please send an email to provisioning@exetel.com.au about this with your mobile service number.
