Since I first signed with Exetel I have changed my plan several times. In my members facilities it says,
Service Activation Date: 2008-12-05
Contract Period: 12 Months
Initial Contract End Date: 2009-12-05
So this means i'm currently out of contract even though I just recently changed plans?

A change of plan does not incur a contract renewal, only infrastructure changes do, for example changing from ADSL1 Telstra to ADSL2 Optus.
