in the settings have you got 'firewalled' set to on or off? Have you tried both and also restarted the app after changing the settings?

I tried that can receive and make calls but no one can speak or hear.
Should I delete the app and re purchase it?

i have the same prob, i just setup my 3gs and when the other side picked up i cant hear anything, they cant hear me as well. i tried to turn on and off the fire wall and restart the app and that didnt work. I used 3g to test and using the same voip number i use for the house phone to setup my iphone.

With voip on mobile, can a person call my mobile using the voip number? or is it only for the moble to call out?
