Unlock and Activate not working

Hi, ordered a new sim for mobile phone which we received in only a couple of days but after putting it in and logging back into the Exetel portal then clicking on the Unlock and Active button we get the code ---- punch it in next screen says it's in the process but within 5 minutes it goes back to the Unlock and Activate page!
What's going on? This has been happening for 3 days now!

I have a separate account with a phone for my oldest daughter which we pay for, the phone number is 0497676398 and this is the Customer ID: 355926
